The New Zealand Health Care Home Collaborative is a unique club-funded alliance of PHOs, DHBs and national entities dedicated to supporting the rollout of a new model of primary health care based on Lean thinking, value from a patient perspective and the use of modern technology to support continuity of care. This presentation will provide a brief overview of the model, outline its rapid adoption in New Zealand over the past 2 years, and describe the year 1 impacts.
Martin Hefford has over 20 years’ experience in health services planning and delivery.
Martin has held a number of senior health sector roles, including: GM Integrated Care at Counties Manukau, Chief Operating Officer at Capital & Coast DHB; Group Manager Planning and Funding at Hutt Valley DHB; Senior Locality Manager in the Health Funding Authority; and Group Manager Mental Health in the Central Regional Health Authority. He has also worked as a Senior Advisor for the Department of Social Welfare, and within the NGO sector, and was awarded a Harkness Fellowship in 2013 to work with Kaiser Permanente in the USA. Martin has three grown up children – all boys and all Wellingtonians.
